1. ONEPRINTED COVER LETTER FROM APPLYVISA.UM.DK It must be signed by the applicant. ALL questions must be answered - otherwise the application may not be accepted. |

2. ONE NEW PASSPORT PHOTOGRAPH A passport photo measuring 35mm x 45mm. The photo should bear a close resemblance to your current appearance and you should be facing the camera directly. The distance from your chin to the top of your head should measure between 30mm and 36mm, and your forehead and hairline should be visible. |

3. PASSPORT OR TRAVEL DOCUMENT The document must have at least two blank pages for the visa sticker. The passport/travel document must be valid for at least 3 months after your visit to Denmark. |

4. EVIDENCE OF OCCUPATION/STUDENT STATUS Employment: • Original, recent and signed letter from employer confirming vacation/business trip and possibility of returning to work. Salary slips for the last three months must be included.
Self-employment: • Original, recent signed letter from a solicitor, accountant or Companies House confirming self-employment status.
Unemployment: • Social benefit entitlement letter, if applicable.
Student: • Original, recent, signed letter from the school, college or university confirming course details.
Care giver: • Employment letter from the agency that brought the care-giver to Israel • Letter from and copy of Israeli ID of the employer (the care-taker)

5. PROOF OF PURPOSE OF VISIT
Visit to family or friends: • Online invitation form to be completed by the host in Denmark or original signed invitation letter submitted by the applicant with the visa application. Invitation stating purpose, accomodation and dates of the visit.
Business/Conferences: • An invitation form stating purpose of visit and dates has to be completed by the host company in Denmark and submitted by the applicant together with the visa application.
Tourism: • Confirmed hotel reservation for the entire stay/confirmation of organised tour • If you are planning several visits to other Schengen countries hotel bookings must be produced for all visits. Please note that Denmark must be the main destination. |

6. TRAVEL INSURANCE Proof that you have travel insurance in English. You must be insured for at least €30,000, covering hospitalization abroad, repatriation, medical flight and flight of a corpse. The policy must be valid throughout the Schengen area for the entire visa period. The insurance should be bought in Israel. |

7. EVIDENCE OF SUFFICIENT FUNDS FOR THE VISIT • Original Bank Statement - showing the three months leading up to the date of application. Stamped and signed by bank. (If recent Bank Statement is not available, online printouts or mini statement with original bank letter from the bank showing the name and account number will be accepted).
Business visits: If your employer or foreign business partner is covering your expenses the invitation form must confirm this. |

8. RESERVATION OF RETURN OR ROUND TICKET (We advise you not to buy the ticket before visa has been approved) |

9. NON-ISRAELI RESIDENTS: Residence and re-entry permit for Israel valid at least three months after your departure from Schengen.
Please note that in some cases further documents and guarantees may be required with your application.
An application cannot be accepted if the applicant is not legally in Israel at the time of application. |
